Q Care - Help for People with Physical or Sensory Disabilities

Q Care has been selected by Gloucestershire Supporting People Commissioning Body to provide help for people with physical or sensory disabilities.Gloucester City Homes works in partnership with Q Care to ensure that our residents are given the support that they need to maintain their independence and a service which improves their quality of life.

Q Care provides a number of different services:

Q-Care believe all service users have a right to:

• Be regarded an an individual with unique needs
• Remain in your own home
• Maintain your chosen lifestyle
• Have personal independence and choice
• Have your personal dignity respected
• Have your privacy respected
• Have your cultural, social and religious needs respected
• Have your emotional and spiritual needs respected
• Have access to full information on available services
• Participate in an assessment of your needs

• Receive a non-discriminatory service

• Participate in regular reviews of your needs
• Have access to representation or advocacy as appropriate
• Have access to a formal complaints procedure.
   

Q Care Supporting People Service

The Supporting People programme offers vulnerable people the opportunity to improve their quality of life by providing a stable environment which enables greater independence.

It delivers high quality, strategically planned housing-related services which are cost effective, reliable and complement existing care services.The planning and development of services is needs led.

Examples of service include:

• Supporting someone to shop for themselves is an eligible task ..... directly shopping for someone is not.
• Supporting someone to cook or prepare a meal is an eligible task ......... directly cooking or preparing a meal for someone is not.
   

Q Care Supporting People services - am I eligible? 

Q Care is able to support anyone who is:

• Profoundly deaf
• Deaf / blind
• Hard of hearing
• Visually impaired
• Parents of deaf or blind children.
   

Q Care Supporting People services - What help can Q Care give?

To meet the eligibility criteria you will have a housing related support need where one or more of the following services would be necessary to help develop your capacity to live independently or to sustain your capacity to do so.

General support

• Liaison with statutory and voluntary agencies
• Signposting to specific legal services
• Signposting to local community organisations
• Accompanying you on shopping or housing related errands where you are unable to do alone
• Advice, advocacy and liaison
• Help gaining access to other services.

Housing needs

• Help finding other accommodation
• Help setting up and maintaining home or tenancy.

Abuse

• Help establishing personal safety and security.

Health

• Supervision and monitoring of health and well-being
• Liaising with health professionals and support with appointments.

Mental health

• Emotional support, listening and advice
• Supervision and monitoring of health and well-being.

Financial

• Help managing finances and benefit claims
• Signposting to local community organisations e.g. Citizens Advice Bureau, employment training or education services.

Social

• Facilitating social interaction and overcoming social isolation
• Developing social skills / behaviour
• Peer support

Family and home

• Advice and support on home improvement work (please remember you need Gloucester City Homes approval for home improvements)
• Help maintaining the safety and security of your home
• Developing domestic / life skills
• Home risk assessment

Services which cannot be provided by Q care supporting people services:

• Day to day repairs (these are normally organised by our repairs partner on behalf of Gloucester City Homes)
• Decorating services (in certain circumstances these can be arranged by Gloucester City Homes)
• Domicilliary and home care services (Q Care operates a separate service which is chargeable)
• Gardening (in certain circumstances these can be arranged by Gloucester City Homes)
• Health care (this is usually provided by your local General Practice Doctor, through other health care professionals where necessary)
• Personal Care (this may be available either through Gloucestershire County Council's Children and Young People Service or Adult and Community Service)
• Befriending
• Regular property maintenance services (these are normally organised by Gloucester City Homes)
• Specialist treatment or counselling (this is usually arranged through your local general practice doctor, though other health care professionals where necessary)
• Building works (these are normally organised by Gloucester City Homes)
• Therapeutic / intensive behaviour management
• Cleaning of accommodation.
   

Q Care Supporting People services - How do I get referred or get further information?
